Client Acceptance Form on Docusign

Our goal is to create a more efficient system for handling the Client Acceptance Form within our department and across departments.


INSTRUCTIONS BELOW:

1. Accessing the Agreements Tab:

  • Begin by logging into your DocuSign account.
  • Once logged in, navigate to the top menu and click on the "Agreements" tab. This tab allows you to manage all your documents and templates.

2. Starting a New Document from a Template:

  • In the "Agreements" tab, look for the "Start" button. Click on it to reveal a drop-down list.
  • From this list, select “Envelopes” and then choose “Use a Template”.

3. Selecting the Client Acceptance Form Template:

  • In the templates section, find and open the "Client Acceptance Form Folder" under the Shared Folders section on the left-hand side of the menu.
  • Locate the "Client Acceptance Form" template within this folder. To select it, click the checkbox next to the template name.
  • After selecting the template, click on "Add Selected" at the bottom right corner of your screen. This action will add the template to your new document.

4. Modifying Recipients and Sending the Form:

  • In the "Add Recipients" section, "Endevio Group" is listed as the first signer. To remove this, click on the delete icon next to their name.
  • IMPORTANT: Add your own name as the first signer by entering your details in the recipient field.
  • Once you’ve made these changes, click on "Send Now" to send the form to the intended recipient.

5. Reviewing and Completing the Document:

  • After sending the envelope, you will receive an email notification.
  • Click on "Review Document" in the email. (See below image)
  • On the webpage that opens, follow these steps:
    1. Click on "Continue" to proceed.
    2. Insert the Client's name.
    3. Provide Background Information on the client and attach any relevant documents.
    4. Sign the document and click on "Finish".
  • The document will be automatically forwarded to Cheryl, then Ian, and Bei (in that order) for their signatures.
